Global Burden of Head and Neck Cancer over 30 Years

Introduction: Head-and-neck cancer (HNC) imposes a significant global healthcare burden. Our study aimed to investigate the burden and trends of HNC from 1990 to 2021, across different regions using the sociodemographic index, sex, and age.

Methods: HNC data were obtained from the global burden of disease study 2021. The estimated annual percentage changes (EAPCs) were calculated to assess trends in age-standardized rates.

Results: In 2021, the incidences of HNC were 1,160,696; deaths were 544,223; and disability-adjusted life years (DALYs) were 15,597,835. Age-standardized incidence rate (ASIR) demonstrated an increasing trend with an EAPC of 0.12. The Age-standardized death rate (ASDR) and age-standardized DALYs rate decreased with EAPCs of -0.82 and -0.94 respectively. The ASIR of females exhibited a positive correlation with sociodemographic index. The ASIR and ASDR demonstrated upward trends with age, and the age group of 50 years and above had a heavy disease burden of DALYs. In 1990, 2000, 2010, and 2021, there was a significant difference in the proportion of HNC incidence rates between males and females in the 40-70 year age group, with relevant proportions for females showing an upward trend every decade (P < 0.05).

Conclusions: Our study identified a global increase in HNC incidence alongside reductions in death and age-standardized DALYs rates. However, a troubling rise in HNC mortality occurred in low-middle sociodemographic index regions, notably among females and the elderly. This highlights the critical need for targeted interventions such as enhanced health education, resource allocation, and early treatment to tackle the increasing global burden of HNC.
