Introduction: Jaw-in-a-Day (JIAD) surgery is an advanced technique for reconstructing post-ablative and traumatic maxillofacial defects. It combines vascularized osseous tissue transfer, immediate dental implant placement, and prosthetic rehabilitation in a single surgery. Leveraging computer-assisted design/manufacturing, virtual surgical planning (VSP), and 3D printing, JIAD surgery streamlines care and enhances functional and aesthetic outcomes. This systematic review evaluates outcomes in JIAD surgery while addressing its challenges and limitations.

Methods: This systematic review adhered to PRISMA guidelines. A PubMed search identified 224 articles, with 109 studies retained after screening. Ultimately, 30 studies met inclusion criteria. Data were extracted and synthesized across implant-related, prosthesis-related, surgical, patient-reported, and clinically measured outcomes.

Results: Thirty studies (2009–2024) comprising 560 patients were included. Study designs included case reports (n=15), case series (n=8), and cohort studies (n=7). Implant success rates ranged from 85.4% to 100%, with 93% reporting favorable outcomes. Immediate prostheses enabled early functional and aesthetic rehabilitation. VSP and 3D printing improved implant accuracy (mean deviations: 1.8 mm for mandible, 2.1 mm for bone grafts) and supported osseointegration (torque ≥35 Ncm). Flap viability ranged from 92% to 100%. Patient-reported outcomes (53%, n=16) highlighted high satisfaction with aesthetics and function and improvements in speech, mastication, and quality of life. Early restoration of oral diet and speech was noted in 4 studies. Complications, such as flap failure and peri-implantitis, were linked to comorbidities, prior radiation, and early learning curves.

Conclusions: JIAD surgery is a transformative approach to head and neck reconstruction, achieving high success rates in oral rehabilitation, surgical precision, and patient satisfaction. In selected patients, the single surgical session reduces operative times and streamlines patient care, improving outcomes and accessibility.
