Long Term Outcomes Following Osia Implantation: Our First 125 Cases

Introduction: The Osia active transcutaneous bone conduction implant system has been FDA approved in the USA since 2019 but experienced delayed clinical adoption due to the pandemic and other factors. As such, long term outcomes studies for the device are lacking. In the present study, longitudinal outcomes associated with the Osia implant are reported for our tertiary care academic center.

Methods: Patients who underwent Osia (2 or 3) implantation at our single tertiary academic center from 2019 to present were considered for inclusion. Data extraction was performed retrospectively and included demographics, operative indications, intra-operative findings, complications and audiometric outcomes.

Results: 116 patients and 125 implants were included. Nine patients underwent bilateral sequential implantation. 39.6% were males 60.3% females. Median age was 48.6 years (R: 11-82). The most common types of hearing loss treated were mixed (53%), single-sided deafness (35%), and conductive (12%). Aural atresia was the most common clinical indication in pediatric patients. Mean follow up for the cohort was 2.3 years. 32 patients had a follow up time of 3 years or more. The all-cause adverse event/complication rate in our series was 10.3%. Mean PTA4 gain (.5, 1, 2, 4 kHz) was 29.62 dB. The most common complications were infection (4.3%) and postop pain requiring an ED visit (1.7%). Reoperation was required in 4 patients including three explanations (two for device extrusion, one conversion to a cochlear implant). Mean device usage was 9.6 hours per day.

Conclusions: The Osia implant is associated with good long term outcomes. Infection is the most common complication. Long term usage was good in this series. To our knowledge, this is the largest study to date reporting on Osia 2 outcomes.
