Efficacy and Safety of Blue Light and KTP Lasers for In-Office Treatment of Laryngeal Pathologies

Introduction: Data on the in-office use of 445 nm blue light and potassium-titanyl-phosphate (KTP) lasers are limited. This study was conducted to further investigate the efficacy and safety of blue light and KTP lasers following in-office treatment of laryngeal pathologies.

Methods: This retrospective cohort study included 168 adult patients (75% male, 25% female) with a median age of 64 years (range, 20-91 years) who underwent in-office blue light or KTP treatment for laryngeal pathologies at the Cleveland Clinic between January 1, 2008, to October 31, 2024. Primary outcomes included time to recurrence, defined as first instance of dysphonia, papilloma, leukoplakia, or erythroplakia following a procedure, and side effects of treatment.

Results: 539 procedures (375 KTP, 164 blue light) were conducted. The most common pathologies included laryngeal papillomatosis (33% of patients), mild dysplasia (4%), moderate dysplasia (3%), severe dysplasia (14%), squamous cell carcinoma in situ (CIS) (15%), and invasive squamous cell carcinoma (11%). Among patients who recurred, the mean time to recurrence was 242 days (95% CI, 214-270 days). For patients with leukoplakia, dysplasia, or CIS, the mean time to recurrence with blue light was 137 days (95% CI, 115-158 days); with KTP, the mean time to recurrence was 244 days (95% CI, 194-293 days). The difference in means was statistically significant [t(172) = 3.9, p < 0.001].

Side effects included bleeding < 1mL (34% of procedures), dysphonia (3%), discomfort (2%), hypertension (2%), dizziness (1%), pain (0.4%), tachycardia (0.4%), bleeding >1mL (0.4%), and tremor (0.2%).

Conclusions: Blue light and KTP lasers were used in-office with minimal side effects and delayed recurrence for an average of 242 days, proving to be both safe and effective in an in-office setting.
