PROGRAM DESCRIPTION: Endoscopic sinus surgery (ESS) is a widely performed procedure for treating chronic sinusitis and other sinonasal conditions. While generally safe, complications can occur due to the complex anatomy of the paranasal sinuses and their proximity to critical structures, such as the orbit and skull base. Complications in endoscopic sinus surgery are rare but can have devastating patient outcomes. Since these events are infrequent, it is often difficult for surgeons to feel prepared for these encounters. Mitigating complications requires a thorough understanding of the anatomy and meticulous surgical planning. This panel focuses on enhancing practical knowledge for the prevention and management of such complications. Proper patient selection, perioperative care and intraoperative safeguards further contribute to reducing the risk of complications. The use of organized algorithms to address high-acuity scenarios such as orbital hematoma, CSF leak, and carotid artery injury, systematically will be discussed in detail. By improving preparedness and confidence in recognizing and treating these emergencies, surgeons can significantly enhance patient safety and outcomes. Emphasis is placed on developing a deep understanding of the nuances involved in managing these situations, fostering a proactive and structured approach to ensure optimal care in high-stakes settings.
