Alginate Therapy for Treatment for LPRD Symptoms: A Systematic Review

Introduction: In patients with laryngopharyngeal reflux disease (LPRD) treatment with proton pump inhibitors (PPIs) is highly effective. However, in some patients, acid-suppressive therapy with PPIs is not as successful. Alginates are medications that work through an alternative mechanism by displacing the postprandial gastric acid pocket

Methods: This study performed a systematic review and meta-analysis to examine the benefit of alginate-containing compounds in the treatment of patients with symptoms of LPRD. PubMed/MEDLINE, Embase, and the Cochrane library electronic databases were searched through December 2024 for randomized controlled trials comparing alginate-containing compounds to placebo, or PPIs for the treatment of LPRD symptoms. Non-English studies and those with pediatric patients were excluded. Heterogeneity between studies was estimated using the I2 statistic. Analyses were stratified by type of comparator

Results: The search strategy yielded 151 studies and 8 met inclusion criteria. Six were included (N = 468 subjects). Alginate-based therapies appear to significantly reduce LPRD symptoms based on RSI and RFS scoring. Compared to PPIs, there was no difference between the two groups.

Conclusions: There remains great controversy concerning the added value of alginates for LPRD.
