Quality of Life and Endoscopic Findings in Patients Managed with Implants for Frontal Sinus Disease

Introduction: Chronic rhinosinusitis (CRS) greatly impacts quality of life. Mometasone eluting sinus stents (MESS) and triamcinolone microsponges (TIM) have been suggested to improve patient outcomes when deployed in frontal sinus ostia during endoscopic sinus surgery (ESS). This study investigates factors that impact quality of life and endoscopic findings in patients managed with MESS or TIM

Methods: A retrospective chart review of patients with CRS who underwent ESS and managed with MESS or TIM was conducted. The type of implant used in frontal sinus surgery, patient demographics and comorbidities, perioperative sinonasal outcome test 22 (SNOT-22) scores, Lund-Kennedy (LK) endoscopy scores, frontal ostium stenosis, frontal sinus surgery type and microbiologic data were systematically recorded and analyzed.

Results: 97 patients met the criteria of inclusion with a male to female ratio of 1:0.67, and a mean age of 55.6 ±15.9 years. 53.6% were managed with MESS and 46.4% with TIM. The mean preoperative SNOT-22 score was 42.1 (41.4 vs 42.9 for MESS vs TIM respectively, p=0.687), and the mean postoperative SNOT-22 scores were 28.3, 22 and 28 at the 1st, 2nd and 3rd postoperative visits respectively. The mean LK scores at the 1st, 2nd and 3rd postoperative visits were 5.1, 5.2 and 4.8 respectively. There was no statistically significant difference in postoperative SNOT-22 scores, LK scores and frontal ostium stenosis in patients with different CRS subtypes, frontal sinus surgery type, and patients treated with MESS vs TIM. The number of postoperative sinus infections and postoperative antibiotic use in patients treated with either MESS or TIM did not impact postoperative SNOT-22 scores at any visit (p>0.05).

Conclusions: The management of chronic frontal sinus disease with MESS or TIM improves sinonasal quality of life and endoscopic findings, with no type of implant superior to the other. CRS phenotypic subtypes, extent of frontal sinus surgery, number of postoperative infections and antibiotic use in patients managed with frontal sinus implants do not significantly impact postoperative subjective or objective outcomes.
