Transoral Robotic Surgery for Recurrent Glottic Cancer Involving the Anterior Commissure

PROGRAM DESCRIPTION: About 20% of all glottic carcinomas involve the anterior commissure (AC), and AC involvement is considered a risk factor for local recurrence and poor prognosis. Transoral robotic surgery (TORS) has been developed to provide a panoramic view of the AC and facilitate en-bloc resection of the tumor through multidirectional dissection using endowristed instruments. In this program, we will demonstrate the use of the da Vinci Xi system to excise a recurrent AC cancer with two instruments: Maryland Bipolar Forceps and Monopolar Curved Scissors, without the need for a temporary tracheostomy. The procedure ensures effective hemostasis during deep dissection, and a clear en-bloc resection is achievable through multi-angle dissection of the mucosa and peeling of the perichondrium with Broyle’s ligament from the thyroid cartilage's inner lamina, all under a high-magnification, 30-degree upward endoscopic view.
