Intraoperative Corticosteroids in Stapes Surgery: A Retrospective Study on Tinnitus and Vestibular Outcomes

Introduction: This retrospective study evaluates the impact of intraoperative local corticosteroid application on auditory and vestibular outcomes in stapes surgery. Specifically, the study investigates tinnitus parameters, vestibular function, and patient-reported outcomes using validated tools and audiometric measures.

Methods: This retrospective cohort study, conducted from January 2023 to May 2025 at a tertiary otology center, examined the effect of intraoperative local corticosteroid application on tinnitus and vestibular outcomes in 200 patients with otosclerosis undergoing stapes surgery. Patients were divided into corticosteroid (n=100) and control (n=100) groups. Outcomes included changes in tinnitus severity, vestibular symptoms, and audiometric measures. Independent variables were treatment type, age, sex, and baseline symptoms. Preliminary analysis showed greater improvements in tinnitus and dizziness in the corticosteroid group, with similar hearing gains in both groups.

Results: The corticosteroid group demonstrated significant improvements in tinnitus severity, as reflected by greater reductions in TFI scores compared to the control group (mean reduction: 12.4 ± 3.2 vs. 7.8 ± 2.9, p< 0.001). High-frequency tinnitus improvements were notably higher in the corticosteroid group (p < 0.01). Vestibular symptom resolution was also superior, with a decrease in dizziness scores observed in 82% of patients in the corticosteroid group versus 63% in the control group (p=0.002). Audiometric parameters, including air-bone gap closure and speech discrimination scores, improved similarly in both groups, with no significant intergroup differences.

Conclusions: Intraoperative corticosteroid use in stapes surgery significantly reduced tinnitus severity and improved vestibular symptoms, with no added benefit in hearing outcomes. These findings support its potential for symptom management in otosclerosis, though further randomized trials are needed to confirm long-term efficacy.
