Otolaryngology-Led HPV Vaccine and Oropharyngeal Cancer Prevention Workshops to Improve Vaccine Education in Primary Care

Introduction: Primary care physician knowledge and counseling on HPV-vaccination is critical for reducing HPV-related oropharyngeal squamous cell carcinoma (OPSCC), but this is underemphasized in resident education. We investigated outcomes of innovative HPV vaccination workshops for primary care residents at three academic centers.

Methods: An evidence-based curriculum was developed through a needs assessment and systematic literature review, incorporating expertise from otolaryngology, epidemiology, and behavioral science. The workshop included interactive didactics on HPV, OPSCC, vaccine impact, and patient counseling strategies. The curriculum was implemented by medical students for internal medicine (IM) and family medicine (FM) residents at the University of Michigan (UM) in 2024 after prior successful implementation by ENT residents at UCLA and UCI from 2021-2024. Pre- and post-workshop surveys assessed resident knowledge, comfort, and skills at baseline and two weeks post-intervention.

Results: Pre-workshop survey of 33 residents at UM revealed gaps in knowledge and counseling of HPV. While all participants knew the HPV vaccine prevents cervical carcinoma, only 82% were aware of its efficacy in preventing OPSCC. Additionally, 81.3% of IM and 94.1% of FM residents incorrectly identified cervical cancer as the most common HPV-related malignancy. Comfort with counseling on HPV-related head and neck (HN) issues was low (31.3% IM, 43.8% FM). Pre-workshop knowledge and attitudes were consistent across UM, UCLA, and UCI cohorts. Post-workshop surveys demonstrated significant improvements in HN HPV knowledge (p <.001) and counseling comfort (p=0.04) at UM, consistent with results at UCI and UCLA.

Conclusions: Baseline gaps in HPV knowledge and vaccine counseling across multiple academic centers highlight the need for enhanced resident education. The HPV Vaccine and Oropharyngeal Cancer Prevention Workshop improved knowledge and counseling comfort across three academic centers when led by medical students, supporting its widespread feasibility and value in addressing this educational need.
