Characterizing Subclinical Sensorineural Hearing Loss in Patients with Neurofibromatosis 2 Using QuickSIN Speech-in-Noise Testing

Introduction: QuickSIN speech-in-noise testing has demonstrated efficacy in identifying hearing deficits in patients with sporadic vestibular schwannomas even when conventional audiometric measures were within normal limits. This study aimed to generate preliminary evidence on its feasibility in detecting subclinical sensorineural hearing loss (SNHL) in patients with Neurofibromatosis Type 2 (NF2).

Methods: A cross-sectional pilot study was conducted from October 2024 to January 2025, involving patients with NF2 diagnosis (aged 10+) with at least one hearing ear and age/sex-matched controls in a tertiary care center. Participants underwent comprehensive audiometric testing, measuring pure tone average (PTA), word recognition in quiet (WRQ), Speech Intelligibility Index (SII), and QuickSIN, which assessed speech recognition in noise. Audiometric outcomes and inter-aural asymmetry were compared between NF2 patients and controls.

Results: A preliminary analysis included a total of 22 ears from 17 NF2 patients alongside matched controls. No significant differences were found in PTA, WRQ, or SII between cohorts. However, NF2 patients showed significantly greater QuickSIN SNR loss compared to controls (4.07 ± 3.62 dB vs. 1.66 ± 1.71 dB; p = 0.01). Inter-aural asymmetry in QuickSIN scores was higher in NF2 patients with intact bilateral hearing compared to control patients (2.80 ± 2.25 dB vs. 1.26 ± 1.03 dB; p = 0.001). No significant differences were observed between ears with and without VS for NF2 patients.

Conclusions: QuickSIN testing effectively discriminates subclinical hearing loss in NF2 patients with normal conventional audiometric measures, highlighting its potential as an early detection and monitoring tool for hearing deficits in this population.
