Parathyroidectomy: Localized versus Bilateral Exploration for Primary Hyperparathyroidism, a Discussion

PROGRAM DESCRIPTION: This session will use a case-based format to compare the approaches of localized parathyroidectomy versus bilateral neck exploration for primary hyperparathyroidism, including the preoperative and intraoperative decision-making processes that are useful to determine which approach is the best course of action to obtain cure. This panel will discuss the utility of preoperative imaging for localization, review of the biochemical labs including calcium and parathyroid hormone levels preoperatively and the role of intraoperative parathyroid hormone and intraoperative pathology to help in assisting decision making in surgery so that post op cure is achieved. Finally, we will review literature looking at the role of adenoma vs four gland disease in certain case scenarios in order to help guide the surgical approach.

OUTCOME OBJECTIVE 1: Compare bilateral neck exploration and localized parathyroidectomy for primary hyperparathyroidism

OUTCOME OBJECTIVE 2: Explain a systematic operative approach given a specific case scenario for bilateral neck exploration over localized parathyroidectomy.

OUTCOME OBJECTIVE 3: Explain how different versions of hyperparathyroidism can guide surgical approach and favor a bilateral neck exploration

BACKGROUND STATEMENT: This panel will fulfill the need to have an algorithm to guide bilateral neck exploration in primary hyperparathyroidism. We will specifically review normocalcemic hyperparathyroidism, and normohormonal hypercalcemia.
