Survival Analysis in Sinonasal Basaloid Squamous Cell Carcinoma

Introduction: Sinonasal basaloid squamous cell carcinoma (SBSCC) is a rare and aggressive malignancy. This study analyzes survival outcomes in patients with SBSCC, focusing on the impact of various clinicopathological factors. Current treatment paradigms involve surgery with optional adjuvant chemoradiation.

Methods: Patients diagnosed with SBSCC in the National Cancer Database were analyzed from 2010-2020. Kaplan-Meier survival analysis was performed to assess the impact of sex, race, co-morbidity, income, education level, insurance status, tumor size, stage, primary site, primary surgery, primary radiation, primary chemotherapy, and adjuvant radiation on survival. Cox proportional hazards regression was used to determine independent predictors of overall survival.

Results: The NCDB contained 513 patients with SBSCC. Stage IV patients had significantly lower 5-year survival (29.9%) versus Stage I (63.2%; p< 0.001). Larger tumor size (>4cm) was associated with worse outcomes, with a 10-year survival of 40.6%. Sinus tumors had higher mortality than nasal cavity tumors (HR:1.79, p=0.021). Male patients had a higher risk of death compared to females (HR: 0.67, p=0.027). Patients residing in areas with a higher percentage of the population without a high school degree had a significantly increased hazard ratio (HR: 2.05, p=0.007). Patients in the highest income quartile had a significantly decreased hazard ratio compared to those in the third income quartile (HR: 0.61, p=0.048). Primary surgery (HR:0.42, p=0.002) and radiation (HR:0.52, p=0.035) were associated with reduced mortality.

Conclusions: Advanced stage, male sex, sinus location, and lower socioeconomic status predict poorer SBSCC survival. Surgery and radiation are both associated with decreased mortality, highlighting the need for equitable access to multimodal therapy and socioeconomic disparity mitigation.
