Isolated Microtia and Its Associated Anomalies

Introduction: Microtia is a congenital condition that can present in isolation or with other systemic anomalies. There is a lack of standardised guidelines for screening of associated systemic anomalies. This paper aims to systematically review existing literature on non-syndromic microtia with systemic anomalies to propose a set of screening recommendations for clinicians when encountering patients with microtia.

Methods: The OVID Medline, EMBASE and Cochrane Library databases were searched for articles relating to microtia and associated systemic anomalies from inception to 23 March 2024. We examined the prevalence of systemic anomalies and the screening protocols employed in the studies reviewed. The methodological quality of the studies was assessed using the Joanna Briggs Institute (JBI) critical appraisal tools.

Results: The search yielded 2727 articles, of which 22 articles satisfied the predefined inclusion and exclusion criteria. Case series, case-control, and cohort studies were included. There were 6002 patients with microtia, of which 1362 (22.7%) had systemic anomalies. The most commonly encountered systemic anomalies were vertebral (37.4%) and facial anomalies (37.5%), in keeping with microtia being linked to, and also considered a mild form of hemifacial microsomia. Other detected anomalies include cardiac (26.4%), neurological (10.1%), eye (9.6%), and urogenital (6.7%). The common diagnostic investigations performed were echocardiography, chest imaging, renal ultrasonography, vertebral imaging, neurological studies and facial imaging.

Conclusions: This systematic review highlights the potential advantages of implementing screening practices to identify systemic anomalies in patients with non-syndromic microtia. Greater standardisation of screening protocols could improve clinical management and outcomes for patients with microtia.
