Does the Timing of Cleft Lip Repair Correlate with Hypertrophic Scar Formation?

Introduction: While cleft lip repair is typically performed between 3 and 6 months of age, earlier repair may lead to better functional and aesthetic outcomes. However, no studies to date have determined if earlier intervention leads to a decrease in hypertrophic scarring or revision surgery.

Methods: Patients undergoing cleft lip repair were queried using associated ICD-10 and CPT codes in the TriNetX database. Chi square and linear regression analyses were used to determine the relationship between age (by month) at time of primary lip repair and the rates of hypertrophic scar formation and revision surgery.

Results: A total of 5838 patients underwent cleft lip repair. Patients undergoing repair in the first three months were less likely to have hypertrophic scar formation (RR 0.59, 95% CI 0.46-0.77, p=0.0001). The month of age at the time of primary repair was positively correlated with an increased rate of scar formation (r=0.77, p=0.025). On the other hand, repair in the first three months was associated with higher rates of revision surgery (RR 1.67, 95% CI 1.38-2.03, p< 0.0001), and the month of age was negatively correlated with rate of revision surgery (r=-0.92, p=0.009).

Conclusions: While earlier cleft lip repair may lead to better aesthetic outcomes, with lower rates of hypertrophic scar formation, it counterintuitively leads to higher rates of revision surgery. These considerations can help to aid in the counseling and decision-making for patients with cleft lip.
