Three-Dimensional Printing as a Perioperative Tool in Rhinoplasty

Introduction: Three-dimensional (3D) printing is an innovative technology due to its customizability, personalization to individual anatomy, ability to be sterilized, and relative affordability. There are numerous applications in the field of rhinoplasty including preoperative counseling, intraoperative surgical guides, and customized nasal splinting. While there are several studies reporting use of 3D printing, this is the first study to critically analyze implementation of this technology and provide recommendations to integrate this tool into practice.

Methods: PubMed, Cochrane, EMBASE, Scopus, and Web of Science were searched systematically for all studies utilizing 3D printing for perioperative use in patients undergoing rhinoplasty. The primary outcomes were the role and characteristics (e.g. material, design) of 3D printed technology. Secondary outcomes included patient/provider satisfaction, design/printing times, operative times, cost, complications, and follow-up. Statistics were analyzed in IBM SPSS.

Results: 17 articles (n=302 patients) met inclusion criteria, reporting use of 3D printed technology for pre-surgical evaluation (6 studies), intraoperative guidance (10 studies), surgical implant creation (3 studies), and post-operative splinting (2 studies). Average total design time was 1.1 ± 1.3 hours, printing time was 79.7 ± 63.6 hours, and overall cost was $145. The most used material was polylactic acid. 3D technology was to create cutting guides/molds (7 studies), full face models (5 studies), create external dorsum templates (3 studies), and external nasal splints (2 studies). Patient satisfaction was overall high and there were no complications at an average follow-up of 7 months.

Conclusions: 3D printing has numerous applications to improve patient satisfaction, augment surgical technique, and accelerate postoperative results in rhinoplasty. 3D printing can also be used during preoperative discussions to share baseline and proposed results, which can improve overall patient satisfaction.
