Salivary Gland Cancers: Management Updates for the Community Otolaryngologist

PROGRAM DESCRIPTION: Salivary gland malignancy is commonly encountered in routine otolaryngology practice. Although surgery forms the mainstay of the management, knowledge of different histologies and their prognosis is paramount to optimize the management of these patients. This includes the extent of surgery, management of the neck nodal basins, referral for adjuvant therapy and also the use of targeted therapy in certain cases. Recently, the WHO classification of salivary gland tumors were updated based on the prognosis of these cancers. It is important to understand these changes as they influence the management strategies and ultimately the oncologic and functional outcomes in these patients.

This panel will focus on the management algorithm of salivary gland malignancy for the community otolaryngologist. This will include recent changes being rolled out for FNA reporting of salivary gland tumors (Milan system) and management of low and intermediate grade salivary gland malignancy. Each panelist will present a 10-12 minutes talk and 20 minutes will be dedicated for case based discussion of commonly encountered scenarios in the work up and management of salivary gland tumors.

OUTCOME OBJECTIVE 1: Understand the changes made to the most recent WHO classification and reporting of salivary gland malignancy as well as the NCCN guidelines of management.

OUTCOME OBJECTIVE 2: Understand the algorithm of management of low and intermediate grade salivary gland malignancy.

OUTCOME OBJECTIVE 3: Recognize the indications of referral for adjuvant therapy in salivary gland malignancy.

BACKGROUND STATEMENT: Salivary gland malignancies are a myriad histologies with differing prognoses. Certain histopathologies have undergone reclassification. Surgery is the mainstay of the treatment for salivary gland malignancies with certain high risk histologies needing adjuvant therapy.
