Exploring Trends in Congenital Cytomegalovirus Care Following Mandated Universal Screening in Minnesota

Introduction: Congenital Cytomegalovirus (cCMV) is a common cause of congenital hearing loss. We investigated trends in evaluation and management of patients with cCMV at a major pediatric center in Minnesota following the state’s implementation of the first universal cCMV screening program in the U.S. in February of 2023.

Methods: This cohort study examined pediatric patients with a diagnosis of cCMV seen between January 2021 and December 2023. Three cohorts were generated based on the year of diagnosis: 2021 and earlier (prior to screening guidelines), 2022 (screening strongly recommended), or 2023 (universal screening mandated). Data on imaging, consultations to Otolaryngology (ENT) and Infectious Disease (ID), treatment with antiviral therapy and hearing technology (HT: cochlear implant and hearing aid) were collected. Chi squared analysis was used to compare the cohorts.

Results: 131 patients met inclusion criteria. The number of diagnoses of cCMV were 3.3 per year prior to 2022, 16 in 2022, and 61 in 2023. There were no significant differences in sex, race, insurance type, comorbidities, or NICU length of stay between cohorts. Comparing the 2022 and 2023 cohorts, there were significant increases in the absolute number of patients undergoing ID consultation (11 vs 24), ENT consultation (9 vs 22), imaging (5 vs 7), and treatment with antiviral therapy (7 vs 9) (p < 0.01 for all), but no difference in the number of patients fit with hearing technology (5 vs 5). There were significant decreases in the percentage of patients diagnosed with cCMV who underwent ID consultation (85% vs 56%), ENT consultation (69% vs 42%), imaging (39% vs 13%), were fit with HT (42% vs 10%) and received antiviral therapy (47% vs 21%) (p < 0.01 for all).

Conclusions: Implementing universal cCMV screening at a major pediatric center in Minnesota increased the number of patients diagnosed with cCMV and increased the absolute number of patients undergoing imaging, consultation to ID and ENT, and treatment with antivirals. However, the percentage of patients who received imaging, ID and ENT consultation, antiviral therapy treatment, and were fit with HT was lower.
