Mandibular Fixation and Plating in a Live Saw Bones Lab

PROGRAM DESCRIPTION: Plating technology has dramatically improved the ability to repair maxillofacial bony trauma with optimal outcomes. A plethora of techniques allow for the rapid reduction and reconstruction of traumatic bony defects at various anatomic sites. Familiarity with these reconstructive paradigms is usually gained during residency through a variety of live hands-on workshops known as saw bone labs. These workshops use fabricated skulls and mandibles to simulate various fracture patterns or ablative defects. A variety of plates in different sizes, strengths, and thicknesses are utilized to reconstruct and repair the simulated defects. Similar to the temporal bone lab, simulation in this setting allows for the development of a technical skill set in a practice environment.
This saw bones lab will simulate a variety of common and unusual mandibular fractures that can be encountered in everyday practice. Plating systems will be available to practice hands on reduction and internal fixation of these simulated fractures. The course will start with MMF using IMF screws/Wave plate technology/Interdental wiring, then proceed with Champey plates for angle fractures, condylar reduction and plating, and finally symphyseal fractures with plates and lag screws. A diverse multi-institutional faculty will permit a unique educational experience given the extensive and variety of experiences. Two attendees per workstation will have access to a broad range of skulls, mandibles, and technical instrumentation. This simulation course can be utilized to develop a new skill set or refresh old skills. There will be one instructor per station.
This course was previously presented and pre and post surveys were performed to determine the efficacy of this learning modality. There was a significant change in all parameters measured which will be presented at the annual meeting. Building on this the use of patient-specific plates for trauma has just entered the field of otolaryngology. We will be integrating this into this particular course to teach a new methodology.

OUTCOME OBJECTIVE 1: Demonstrate understanding of how to obtain maxillomandibular fixation in the trauma setting.

OUTCOME OBJECTIVE 2: Demonstrate understanding of current paradigms in the management of mandibular fractures. Integrate PSP and 3D modeling into Trauma care

OUTCOME OBJECTIVE 3: Attendees will be able to utilize contemporary plating technology to reduce and reconstruct traumatic bony disruptions of the mandible using plates and lag screws..

BACKGROUND STATEMENT: current management paradigms for facial trauma require familiarity with a rapidly changing technological profile of plating systems. This simulation course will provide a hands on setting to maintain, refresh or develop a new skill set
