When Trial Data Meets Reality: Comparing Advanced Laryngeal Cancer Outcomes from TCIA and Northern-England Cohorts

Introduction: While clinical trials provide critical insights into treatment for advanced laryngeal cancer (ALC), they often fail to reflect real-world outcomes, particularly in populations with greater comorbidities and socioeconomic challenges. The Cancer Imaging Archive (TCIA), a repository of trial-based imaging and outcome data, represents highly selective cohorts. In contrast, Northern England’s Newcastle cohort includes a real-world population with significant health and social complexities. This study compares survival outcomes to assess how well trial-derived data translates to real-world scenarios.

Aim: To evaluate whether TCIA trial data reflects real-world outcomes in Northern England’s ALC cohort.

Methods: A retrospective analysis compared the Newcastle (n=222) and TCIA (n=196) cohorts. Kaplan-Meier survival curves and Cox regression analyzed differences in demographics, treatments, and 5-year disease-specific survival (DSS). The Newcastle cohort represented a real-world population with higher comorbidity burden and a predominance of surgical interventions, while TCIA reflected trial-based cohorts with more chemoradiotherapy.

Results: The Northern England cohort had significantly lower 5-year DSS (66.8% vs. 75%, p< 0.001). Surgical treatment with adjuvant therapy was more common in Newcastle (51%), while chemoradiotherapy (53%) and radiotherapy alone (41%) predominated in TCIA. Kaplan-Meier analysis showed significant survival discrepancies (log-rank p< 0.01), driven by Newcastle’s poorer general health and socioeconomic challenges.

Conclusions: This study highlights the limitations of generalizing trial data to real-world populations, such as those in Northern England, where survival outcomes are shaped by complex health and social factors. Bridging this gap is essential for equitable and effective treatment strategies.
