Gender-affirming Facial and Vocal Surgery

PROGRAM DESCRIPTION: The face is one of our most important interactive means, as it carries social meaning, facilitates non-verbal communication, and enables a self-identity that includes gender-identity. In gender-affirming facial surgery, gender-typical characteristics are altered to reflect gender identification. This course aims to discuss the differences between the male and female face, and the specific surgical techniques used to alter the sex of the face and voice.

Surgical discussion topics will address commonly utilized techniques in gender affirming surgery of the face with a focus on facial feminization surgery. Specifically, brow reduction and lift, hair line alteration, frontal table set back, mandibular reduction, and feminizing rhinoplasty, will be described. Voice therapy and surgery techniques to feminize the voice and alter the thyroid cartilage will be discussed. Importantly, discussion will center on the role of otolaryngology- head and neck surgeons to provide these services to the underserved transgender community.

OUTCOME OBJECTIVE 1: Appreciate the challenges transgender patients encounter obtaining medical and surgical care. Understand the importance of creating a gender non-biased clinical environment for patients.

OUTCOME OBJECTIVE 2: Identify facial characteristics of the male and female face and recognize areas of the face that can be altered. Appreciate and understand the surgical techniques used to perform gender affirming surgery of the face and neck.

OUTCOME OBJECTIVE 3: Appreciate and understand the role and indications for voice therapy and voice surgery.

BACKGROUND STATEMENT: Otolaryngologists can provide essential services to help the transgender community, particularly with voice and facial surgery. Members should be well aware of this underserved community. The emphasis of this presentation will be on patient evaluation and surgical techniques.
